Our referral process

We aim to make referral conversations clear, responsive and practical. Our pathway helps professionals and families share the right information so we can review needs, discuss suitability and agree next steps promptly.

Referral enquiry received

Referrals can be made for adults aged 18 and over who may benefit from specialist supported living and mental health support.

Referral information shared

To support a clear review, please include referrer details, preferred borough and service need, support needs and risks, and intended outcomes.

Needs reviewed

We are currently open to referrals and able to review needs within 48–72 hours.

Viewing and suitability discussion

Where appropriate, we may invite the individual, their family or involved professionals to view the service so we can help assess whether the environment is suitable, appropriate and likely to feel comfortable for the person.

Suitability and comfort review

We review whether the environment, support pathway and level of input are likely to meet the person’s needs and whether the placement is likely to feel safe, appropriate and manageable.

Next steps agreed

Following review, we provide clear feedback on likely fit, any further information needed and the next steps for progression, further discussion or transition planning.
